    Plaquenil use with retinopathy

Pattern of Retinopathy: Although the locus of toxic damage is parafoveal in many eyes, Asian patients often show an extramacular pattern of damage. Dose: We recommend a maximum daily HCQ use of 5.0 mg/kg real weight, which correlates better with risk than ideal weight.

Hydroxychloroquine Plaquenil; HCQ has been an important and effective drug for the treatment of lupus erythematosus and related autoimmune and inflammatory diseases for half a century, although its potential to cause retinal damage continues to raise concern among rheumatologists and ophthalmologists. Use of Plaquenil is contraindicated in patients with known hypersensitivity to 4-aminoquinoline compounds. Warnings Resistant strains of malaria Plaquenil is not effective against chloroquine-resistant strains of P. falciparum see CLINICAL PHARMACOLOGY – Microbiology. Studies have shown that patients who have manifested with retinopathy have traces of Plaquenil metabolites in their erythrocytes, plasma, and urine even after 5 years of discontinuing the medication 1, 2, 3.

Risk of Toxicity: The risk of toxicity is dependent on daily dose and duration of use. There are no similar demographic data for CQ, but dose comparisons in older literature suggest using 2.3 mg/kg real weight.

Is extreme fatigue a side effect of Plaquenil? Hi! I think Plaquenil is the cause of my extreme fatigue too. I am not anemic at this time and I get exhausted just trying to get through the day. I have also been on Plaquenil about three months. All of my arthritis swelling and pain are gone, but the fatigue will not let up! Hope things change for you.
