Tell your doctor and pharmacist about all of your drugs (prescription or OTC, natural products, vitamins) and health problems. You must check to make sure that it is safe for you to take chloroquine with all of your drugs and health problems.

Chloroquine also is taken up into the acidic food vacuoles of the parasite in the erythrocyte. It increases the pH of the acid vesicles, interfering with vesicle functions and possibly inhibiting phospholipid metabolism. In suppressive treatment, chloroquine inhibits the erythrocytic stage of development of plasmodia. Chloroquine enters the red blood cell by simple diffusion, inhibiting the parasite cell and digestive vacuole. Chloroquine then becomes protonated to CQ2+, as the digestive vacuole is known to be acidic pH 4.7; chloroquine then cannot leave by diffusion.

For all uses of chloroquine: WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Do not start, stop, or change the dose of any drug without checking with your doctor.

A severe eye problem has happened with chloroquine. This may lead to lasting eyesight problems. The risk may be higher if you have some types of eye or kidney problems. The risk may also be higher with some doses of chloroquine, if you use chloroquine for longer than 5 years, or if you take certain other drugs like tamoxifen. The scientists concluded by stating “Our findings reveal that remdesivir and chloroquine are highly effective in the control of 2019-nCoV coronavirus infection in vitro. Since these compounds have been used in human patients with a safety track record and shown to be effective against various ailments, we suggest that they should be assessed in human patients suffering from the novel coronavirus disease.” The 4-aminoquinoline antimalarials are membrane-soluble weak bases known to accumulate in malaria-infected erythrocytes,11 and it is therefore relevant that the parasite digestive vacuole lysosome, the site of haemoglobin digestion, where the first effects of chloroquine are seen12 and which has been proposed as the main site of drug uptake,13 is enclosed by a lipoprotein membrane, and has an acidic pH around 5.2.14 At this pH, the 4-aminoquinolines will be di-protonated doubly positively.

Plaquenil - Intestinal problems should resolve pretty quickly, but I do have ongoing gas/minor discomfort from plaquenil. Beano and Gas-X help a lot with that. It's a small price to pay for the major positive impact of the drug, for me. I'm on 400 mg/day. Trying out different times of the day and/or taking it after a full meal might help.
